The Case Management Nurse will demonstrate excellent interview and assessment skills, strong organizational skills. He/she will be involved in collaboration with other FHCP departments and outside facilities/companies in the care/service coordination of patients. The main focus of this position will be to support the department in areas of UR/Triage, Complex Program Coordination and Skilled Care Coordination. Assignment to each area as department needs necessitate; requiring staff this position to be self motivated, flexible and able to maintain up to date knowledge in each area.
