Responsibilities: Obtain anticoagulation-related history and physical data on patients. Develop plan of care including medication adjustment, and communicate plan to referring physician for follow-up. Order appropriate laboratory tests. Manage anticoagulation, including change medication dose and/or frequency. Educate patient regarding disease and treatment, including medication administration, signs and symptoms of untoward effects, factors influencing response to therapy, and needed lifestyle changes based on the patients physical, psychosocial, educational and related criteria appropriate to the age of the client served. Provide telephone triage and telephone advice to patients undergoing anticoagulation. Maintain and review patient records, charts, and other pertinent information according to Anticoagulation Clinic Standards.
