Uses clinical judgment skills to diagnose and treat patient responses to actual or potential health conditions. Assesses, monitors, detects, plans and takes appropriate action to prevent potential physiological complications and promotes optimal level of health. Accountable for integrating care within scope of practice licensure, relevant education and experience. Reports to shift Team Leader and is accountable to the Clinical Nurse Manager. Assesses and monitors total physical, psychological, sociocultural and spiritual needs of patients/significant others. Develops, implements and evaluates an individualized written plan of care based upon a physiological, psychosocial, spiritual assessment consistent with patient/significant others' changing needs and goals. Integrates care in collaboration with other members of the health care team. Participates in transition planning, in collaboration with health care team, at time of admission and continues plan through discharge. Provides competent care consistent with the nursing process and written plan of care, physician's orders and in accordance with hospital policies and procedures. Assesses patients and significant others educational needs and develops and implements and evaluates appropriate teaching strategies. Documents care consistent with guidelines. Demonstrates clinical competence in drug therapy, pain management utilizing protocols, procedures, policies and technology available. Maintains a safe work environment by adhering to hospital and departmental policies and procedures. As listed in specific unit performance plan and or Competency Based Objectives.
